Please read the Electronic Signature Agreement below and then complete the Scheduled Planned Outage Request Form. Upon clicking "finish," your form will be submitted to our Service Department for final processing. 

NOTE: If you do not receive a return call after submitting this form within a considerable amount of time from a BrightRidge Service Department Representative, call (423) 952- 5000, Monday - Friday 8am to 5pm for confirmation as to your request.

*Unforeseen circumstances; such as, a widespread outage due to weather, may delay reconnect of service.
